Day of FREE Health Screenings and Dental Care October 24, 2009

Presented by Highmark Partners for a Healthy Community Erie. Sponsored by Highmark Blue Shield and United Concordia dental insurance in collaboration with Booker T. Washington Center.

* Blood Pressure
* Cholesterol
* Vision
* Flu Shots

Free Dental Services for Children and Adults:

* Screenings
* Cleanings
* Simple extractions and fillings

Please note that space is limited. You may not receive all screening/procedures the day of the event. To be eligible for screenings, you must be uninsured or have minimal health and dental coverage.

In addition to these free services, insurance counselors will also be available. Please bring:

* Social Security number
* Photo ID
* Proof of income for last four weeks.
